MNB Computers & Electronic Repair

1. Authorization: I authorize MNB Computers & Electronic Repair to perform all necessary diagnostic and repair services on the device listed above.
2. Data Responsibility: I understand that I am solely responsible for backing up all data. The shop is not liable for any data loss, corruption, or damage that may occur during diagnosis or repair.
3. Estimates: Any estimate provided is approximate. Additional parts or labor may be required. I will be contacted for approval before work exceeding the original estimate is performed.
4. Payment: Full payment is due upon completion of services before the device is released. Accepted payment methods include cash, card, or other methods as agreed.
5. Warranty: Repairs carry a limited warranty of 30–90 days on parts and labor (details provided at completion). This does not cover damage from misuse, accidents, or software issues after the device leaves the shop.
6. Abandoned Property: Devices left unclaimed for more than 30 days after notification of completion may be considered abandoned. The shop reserves the right to recycle or dispose of the device to recover outstanding costs.
7. Liability: The shop’s liability is limited to the cost of the repair service. We are not responsible for any consequential damages.
8. No-Fix Board Repair Fee: All PC board work will have a minimum of $100.00 fee for attempted but unsuccessful board repair.
9. Shipping Costs: Customers are required to pay for all shipping costs to and from MNB Computers. If a warranty claim is made and approved, MNB Computers will reimburse the shipping costs.
10. No Signature: Customers are required to Print, Sign, and include this form with all mail-in repairs or you will be billed a return shipping fee to send the item back.
11. Prior Repair or Damage: Devices that have been previously repaired, modified, liquid damaged, physically damaged, or tampered with may experience additional failures during repair. The shop is not responsible for pre-existing conditions or latent defects discovered during service. This must also me disclosed prior to shipping to us.
